How can I get more Curriculum Report Forms (CRFs)?Right on our website! Under the Educator heading on the left-hand toolbar, there is a page titled “Implementation Forms”. You can download and print forms from there.

What do I do with completed CRFs and completed pre- and post-tests?Send all completed CRFs and pre/post tests to Heather Bjur at the Make a Sound Choice office. Remember: completed CRFs trigger your section disbursement checks! Fill them out completely and correctly – that will make us so happy.

Why is my section disbursement smaller this year (9/30/09 – 9/29/10) than it was last year? As announced at the June 2009 2-day and Booster Training events, all CBAE awards have been reduced for the upcoming grant year. The current administration in Washington, DC has announced its intent to eliminate funding for CBAE grants from upcoming federal budgets.
