How can I get more Curriculum Report Forms (CRFs)? Right on our website! Click ‘Educators’ on the home page and then click ‘For Educators Who Are Implementing’ and then click ‘Forms.’ You will be able to download and print all forms from there.
What do I do with completed CRFs and completed pre- and post- tests? Send all completed CRFs and pre/post tests to the Make a Sound Choice office. Please fill them out completely. Our mailing address is on the back side of the CRF.
